Medical Consultation Invoice Template
GP/specialist invoice with Medicare item numbers. Example: Item 23 Long consultation $78.50, Gap $39.25.
What a medical invoice must carry
- The provider number for the location where the service was rendered
- The practitioner’s name, and the patient’s name and date of birth
- The date of service, one line per service
- The MBS item number for each service, with a short description
- The fee charged, the Medicare benefit if known, and the gap paid
- Referral details and referral date for a specialist attendance
- Your ABN and practice contact details
Item numbers are the whole document
A medical invoice is read by Medicare or a fund, and assessed against the item number and the service date. The descriptive text is a courtesy; the item number is the claim. Where a referral applies, its date sets the period the referral covers, which is why it belongs on the invoice rather than only in the file.
- Bill the item that matches the service actually provided, including the consultation length where the item depends on it
- Never merge two same-day services onto one line — they are assessed separately
- Show the gap clearly, because that is what the patient is out of pocket
Patient-facing practicalities
- Tell the patient at booking if the practice does not bulk bill, and what the likely gap is
- Issue the invoice at the time of the attendance, while the detail is correct
- Keep clinical detail on the invoice to the minimum the claim needs
