Allied Health Invoice Template
Physio/chiro/psychology invoice. Example: Initial assessment $120, Follow-up treatment $85.
What an allied health invoice needs on it
- Your provider number, and the practitioner’s name if the practice bills under several
- The patient’s name and date of birth, which private health funds match on
- The date of each attendance, listed separately — funds reject invoices that merge a block of sessions
- The item number for each service, and a plain-English description beside it
- The fee charged, any amount already paid, and the balance owing
- Your ABN and practice contact details
- A referral date and referring practitioner, where the service was referred
Making the invoice work as a rebate claim
Most allied health invoices are handed straight to a health fund or uploaded to a Medicare claim, which means the patient is not really your audience — an assessor is. Funds decline on detail, not on substance: a missing provider number, a service date that reads as a range, two sessions on one line. The patient then calls you, not the fund.
- One line per attendance, never “6 sessions, March”
- Item numbers exactly as published — a transposed digit reads as a different service
- Show the gap the patient actually paid, so they can claim the right amount
Privacy on a document that leaves your practice
- Include only the clinical detail the claim requires — an invoice is not a progress note
- Send it to the patient, and let them pass it to the fund, unless they have asked otherwise in writing
- Store it under the same retention rules as the rest of the clinical record
