Dental Invoice Template
Dental services invoice with item codes. Example: Examination $65, Scale & clean $150, X-rays $95.
What a dental invoice should itemise
- The provider number, and the dentist who performed the treatment
- The patient’s name and date of birth
- The date of service for each item, listed separately
- The ADA item number for each procedure, with a plain description
- Tooth number or quadrant where the item requires it
- The fee per item, any amount paid on the day, and the balance
- Your ABN and practice details
The invoice is usually a health fund claim in disguise
Patients hand a dental invoice to their fund and expect a rebate within days. Funds assess on item numbers and dates, and they decline quietly — the patient finds out weeks later and calls the practice. Getting item numbers and tooth notation right on the invoice removes almost all of that follow-up work.
- One line per item per tooth, even where the same item is repeated
- Use the current ADA schedule — item numbers change and funds price against the current list
- Show the gap the patient paid, because that is the figure they need to claim
Quotes for treatment plans
- Give a written plan with item numbers before major work, so the patient can get a pre-assessment from their fund
- State clearly that a quote is an estimate and what would change it
- Note where a fund annual limit is likely to be reached mid-plan
